MiVIA
Working with MiVIA and other community based partners, and with support from United Healthcare, CHD has implemented a pilot Personal Health Record (PHR) project in San Joaquin County. Over 2,500 farmworkers and other low income individuals have been enrolled in MiVIA and, in addition to the portable electronic health record, have received an emergency medical card with photo ID that list medications, allergies, conditions and emergency contacts as well as a personal email address. In addition to the portable electronic health record, MiVIA members receive an emergency medical card with photo ID that list medications, allergies, conditions and emergency contacts as well as a personal email address. CHD’s outreach educates farmworkers about health choices and access to care. Through the use of CHD patient navigators or promotores, farmworkers with chronic medical conditions learn how to better manage their conditions (such as diabetes, asthma) and gain a better understanding of their health status using the MiVIA PHR.
